2008 Pre Seminar Materials

All HOBY participants MUST fill out the Pre Seminar Forms and return them to the Idaho Director of Recruitment.  Forms must be returned no later than 5pm on May 2, 2008.

All participants are required to bring a notorized copy of form #5 - Consent & Acknowledgement of Risk Form - to the Seminar Check-In.

Forms should be returned, via email, to recruitment@hoby-idaho.org.  If you wish to return the forms via fax or mail, please contact:
